free cell phones for low income

People are walking the streets and offering free cell phones to anyone, or people that look low income. They have a box in their hand that has a picture of a cell phone on it, or a plain white box and tell you you can have a free phone and free service if you fill out some info.
The person has a tablet in his hands too and needs personal info concerning you being on ssi, ssdi, food stamps, or social security, or welfare. He asks you to prove you qualify by showing your debit card from your direct deposit bank account, or your social security monthly debit card, or your food stamp card.
He puts the info into his tablet and has you sign something on the tablet and also puts in your legal address. He then hands you a box and says put in the battery and make free calls. I got home and put in the battery but I had no sim card and no phone number for ther phone.
I quickly realized I was scammed and canceled my bank debit card. Now I am concerned about identy theft.
